Mojtaba Khamenei was declared the new Supreme Leader of Iran, replacing his father, just days after the assassination attempt that wounded him. He has not appeared in public since. That February airstrike killed his wife, son and his father, former Supreme Leader Ali Khamenei.

Since that day, the only thing Iranians have heard or seen from their new leader are messages supposedly written by him and images that appear to have been modified or generated by artificial intelligence.

This has fueled speculation about his health and his role in the Iranian leadership. On Saturday, for the first time, a Tehran official confirmed that Khamenei had suffered injuries to his kneecap and back, without specifying the severity of his condition.

Khamenei's whereabouts are unknown. He does not use electronic devices to communicate, interacting only with those who can visit him in person or by sending messages through couriers. Khamenei remains in isolation as he continues to receive medical treatment for his injuries, which include severe burns to one side of his body, his face, arm, torso and leg.

However, Mazaher Hosseini, chief of protocol in the office of the Supreme Leader of Iran, stated that he is recovering and "is now in full health."

"People need to be patient. He will speak to them at the right time," he added.

Iranian President Masoud Pezeshkian said earlier this week that he had held a two-and-a-half-hour meeting with Khamenei, the first documented in-person meeting between a senior Iranian official and the new Supreme Leader.

Despite this, US intelligence believes that Mojtaba is playing a key role in defining Iranian military strategy, contributing alongside senior military leaders to decisions that should lead to an end to the conflict, although the exact hierarchy within an already fragmented regime remains unclear. Internal divisions that have hampered Iran's ability to present joint proposals to US negotiators.

After the assassination of Khamenei's father, Trump nevertheless boasted that he had imposed regime change on the Islamic Republic and called those who now negotiate on behalf of Tehran "reasonable."

Ali Vaez, director of the Iran project at the International Crisis Group, had explained that regardless of whether or not the new Supreme Leader was able to lead the talks, “attributing certain positions to him is a good way for Iranian negotiators to protect themselves from criticism.”

It is also possible that some members of the Iranian power structure claim to have access to Khamenei to appropriate his authority and promote their own interests.
